I'm looking through my collection of articles today and found this
one on dumping. I don't remember where I originally found it so
unfortunatly I cannot give proper credit. My apologies for that.
DUMPING SYNDROME DEFINED, EXPLAINED, AND AVOIDABLE!
The weight loss that is achieved through the RNY appears to be
primarily dependent upon the removal of the pyloric valve, leaving
the individual without a regulator of food movement into the small
intestine. This results in the well-known phenomena called "dumping
syndrome" which can cause an individual to feel sick or even faint.
It can also cause you to sweat, shake, feel tired, and sleepy.
Rapid gastric emptying, or dumping syndrome, happens when the lower
end of the small intestine (jejunum) fills too quickly with
undigested food from the stomach. "Early" dumping begins during or
right after a meal. Symptoms of early dumping include nausea,
vomiting, bloating, diarrhea, and shortness of breath.
"Late" dumping happens 1 to 3 hours after eating. Symptoms of late
dumping include weakness, sweating, and dizziness. Many people have
both types.
Stomach surgery is the main cause of dumping syndrome because surgery
may damage the system that controls digestion. Treatment includes
changes in eating habits and medication. People who have dumping
syndrome need to eat several small meals a day that are low in
carbohydrates and should drink liquids between meals, not with them.
It is imperative to the Roux En Y or other procedural gastric bypass
patient that they follow their diet plan to reduce the risk factor of
Dumping Syndrome. Many people equate the term solely to be spasmodic
fits of diarrhea, when that is merely one of the symptoms of a more
intensified unpleasant experience. Dumping syndrome is usually
divided into "early" and "late" phases - the two phases have separate
physiologic causes and shall be described separately.
In practical fact, a patient usually experiences a combination of
these events and there is no clear-cut division between them. Early
dumping is caused by the high osmolarity of simple carbohydrates in
the bowel. The various types of sugar all have small molecules, so
that a gram of (for example) sucrose has MANY more molecules than a
gram of protein, creating a higher concentration (number of molecules
per cc) from simple sugars than from other foods. This matters
because, inside the body, fluid shifts will generally go toward the
higher concentration of molecules. So, if a patient consumes a bite
of milk chocolate (lots of sugar), when it gets to the Roux limb it
will quickly "suck" a significant amount of fluid into the bowel.
This rapid filling of the small bowel causes it to be stretched
(which causes cramping pain). This also causes the activation of
hormonal and nerve responses that cause the heart to race
(palpitations) and cause the individual to become clammy and sweaty.
Vomiting or diarrhea may follow as the intestine tries to quickly rid
itself of this "irritant."
Late dumping has to do with the blood sugar level. The small bowel is
very effective in absorbing sugar, so that the rapid absorption of a
relatively small amount of sugar can cause the glucose level in the
blood to "spike" upward. The pancreas responds to this glucose
challenge by "cranking up" its output of insulin. Unfortunately, the
sugar that started the whole cycle was such a small amount that it
does not sustain the increase in blood glucose, which tends to fall
back down at about the time the insulin surge really gets going.
These factors combine to produce hypoglycemia (low blood sugar),
which causes the individual to feel weak, sleepy, and profoundly
fatigued. Late dumping is the mechanism by which sugar intake can
create low blood sugar, and it is also a way for gastric bypass
patients to get into a vicious cycle of eating. If the patient takes
in sugar or a food that is closely related to sugar (simple
carbohydrates like rice, pasta, potatoes) they will experience some
degree of hypoglycemia in the hour or two after eating.
The hypoglycemia stimulates appetite, and it's easy to see where that
is going.... The reason that sugar does not cause dumping in non-
operated people is that the stomach, pancreas, and liver work
together to prepare nutrients (or sugar) before they reach the small
intestine for absorption. The stomach serves as a reservoir that
releases food downstream only at a controlled rate, avoiding sudden
large influxes of sugar that can occur after a Roux En Y. The
released food is also mixed with stomach acid, bile, and pancreatic
juice to control the chemical makeup of the stuff that goes
downstream and avoid all the effects outlined above.
Obviously, surgeons consider dumping syndrome to be a beneficial
effect of gastric bypass -it seems to be important to provide quick
and reliable negative feedback for intake of the "wrong" foods. In
practice, most patients do NOT experience full-blown dumping more
than once or twice. Most simply say that they have "lost their taste"
for sweets. Of course, this is always a great topic to ask patients
about directly, so you may want to ask about it at our support group
in person, or in a support group online.
It is an unpleasant side effect of this surgical procedure,
experiencing it means one simple thing; you are not following your
prescribed diet, and unless you do, dumping syndrome is going to
become a major part of your life.
Do yourself, and your body a favor and follow the doctor's orders, to
the letter.

I've created a recipe file for all of us to post recipes in. Everyone
is welcome to add to this. I was thinking it would be a great way of
sharing that is a little easier to access than sifting through posts.
I've taken the liberty of adding the quiche recipe (that looked darn
good to me - 5 years out).
I was hoping that once the file has enough recipes that our dietician
could separate them into what's *friendly* to who and perhaps
eventually become printed and available to us.

I had my surgery on 6/6/06 with Dr. Houston. I told my story earlier
but I had about 4 1/2 hrs. of surgery as I had an esophageol hernia
which took over 2 1/2 additional hrs. I was in the hospital for 6
days total.
Again, thanks for your idea...we shall only hope it works. I did
like using the BETA thing so I could have all of those cute little
faces in my postings.

I survived today. [INLINE] Dr. Houston was so great, we talked about the things that had caused me to not do so good and how I could change them. I am soooooooo glad that I went. I talked with a nutrionionist and also set a good exercise program that I can commit too. I got to talk with Kathy H too, and she gave me some more info for my support group. Kathy you are so committed (THANK YOU) [INLINE] . I also found a couple of shirts in the clothes that people have donated , thanks to those who donated [INLINE] .

And what can I say about Dr. Frye.........that woman is aammaazziinngg!!!!!! It really helped to share in a group face to face. I enjoy this forum too, it has helped me change my life.

It was also nice to meet you (Linda T) [INLINE] . I will be going back monthly for a while and attending group then as well.

I have found a subsitute for the orangaide. Walmart brand of crystal
light has a drink mix called sunrise orange? It actually taste better
than tropicana and it's cheaper! You can also take the regular kool-
aide packs(whatever brand)and add splenda and it's cheaper with more of
a variety and better taste. (or you can use whatever sweetner you want)

The italian crustless quiche is an okay recipe however for patients
less than 6 months post-op we recommend no raw or fresh cooked
vegetables. Yes, onions can definitely produce gas and GI distress
and they are considered a non-starchy vegetable so other than that
they are not really any different from the other low carb vegetables
in group B, just that they could cause a lot of discomfort while you
are healing and not far enough out from surgery to tolerate them
well. The cheese called for in this recipe is too high in fat and
calories (even though it is mozzarella) during the weight loss
phase. I know it is hard to find a lot of variety in the beginning
but there just aren't many recipes that are appropriate using the
guidelines in the manual. If it's not in the manual, we don't
recommend it, and your doctor doesn't recommend it. Just experiment
with herbs and spices and the allowed cooking methods, and try to add
variety with the same foods for flavor, i.e. marinade chicken in FF
Italian, mix dill and mustard for flavor with grilled salmon, dip
deli turkey slices in mustard, bake chicken in chicken broth and add
poultry seasoning, etc. Get creative!

I also meant to add to this that I also journal....and it really does

help! But what helps the most is when I go back and read what

I have written in them years later. And I wander, why was I feeling like that....or why did I let that person make me feel

that way about myself! Journal as much as you can!! It really

will help you with all of that emotion that you don't know what to do with. Hope this helps!!

she would really appreciate being a part of your WL. Are you
going to get her to do your measurements too? I'm gonna see
if I can get one of my nieces to do mine, since I don't have any
kids. I don't know about you, but I'm getting excited about the
sugery and becoming a BIG LOOSERRR!!! Hearing all the
group talking and sharing about their experiences is just making
it more real and exciting for me, I can hardly wait! I still have one more month from tomorrow before I go to see Dr. Dyer for
my consultation. Boy, sometimes the time just ticks by and
sometimes it seems to fly by!!

I'm still here. Jill is right, Tim & I are on the road together and are doing great.

over the hump-I think-I need input please

Well I am finally over the so called hump I guess after the 10th day
but for some reason I am wondering if my band slipped. I am usually so
full and uncomfortable after I eat yet this a.m. I drank 1/2 a protein
shake, had one scrambled egg (although I did not eat it all) and have
had 3 bottles of water thus far....and I have not had that pain and
soreness like I did before ....is this normal...is that how it
eventually feels after 10 days?? How do I know if my band slipped
anyway??? Can anyone help with this???

Hi Fran,
This morning I posted an email early this morning before I left for
work on the CMC website saying I 'surrendered' and putting up the
white flag as it's just too much trying to figure out these posting
problems. BUT TO THE BEST OF MY KNOWLEDGE, I HAVEN'T SEEN THE
POSTING YET AND IT'S ALMOST 2:30PM. I've spent hours of my work and
personal time trying to figure this out. I am giving up! I don't
want to be too negative but it's been over 3 weeks with these
problems. I said in my morning email that I would presume it's the
doctors paying for this website and they aren't getting their money's
worth. They should have a Help Desk for their IT folks to solve this

I am finally caught up on my e-mails. Now for some business......I think we should go with Embassy Suites. The rooms are $120 a night BUT they are suites with 2 rooms and beds in both rooms. So we can share rooms and therefore the cost will be $120 for the entire weekend. PLUS, they have some real perks......such as the way the food will work AND the meeting room space necessary as well as a pool, exercise area, etc. I have some ideas. This is definitely cheaper than the Holiday Inn's quoted price. AND we will only need to pay for 3 meals instead of 5 with them. I have some info coming and will send it out when I have it. I am so excited.....especially after last weekend. Can't wait to share it all with each of my buddies.

Thanks Sandi , you are doing wonderful keep up the good work .YES It was a very scary ordeal knowing little Nate was lost out there somewhere , but I am sure that anyone here would put theirself out there to help any way they could my sister is still morbidly obese and even she was bringing coffee and doughnuts to the search team. There are times when we think we can't do the impossible but when push comes to shove we can. The hills and waterfalls and and drop offs as well as old miners pits make where I live a very dangerous area for a grown up who doesn't watch carefully where they are going at night So all I can say is this little boy was cared for by God himself to withstand the cold and the cyotes that night I sure would not have been calm enough to take a nap like he and Arend his dog did :o) I was so happy when my cell rang (thanks to it being a razor ) otherwise it would not have rang because when I got the call Jeremy was with another party looking and my
brother in law was with me and he had NO service with the same provider. My sister tried to call him first after she was lucky enough to witness the park ranger walking little Nate outta the woods as she was going for more coffee .Tammy (sis) was just crying she said she will never forget that scared little boy with the blue hoodie and jeans with rubber boots on walking down the road with the ranger with his hands in his hoodie pockets Bless his heart little Nate said I saw all them lights last night but didn't know what it was . He said me and arend walked most of the night when we gotr so tired we laid down and had a nap and Arend kept me pretty warm

Hon, I felt the exact same way when I had to do the six month thing. But if you do your best to "follow the rules" you'll be amazed at the things that you find out about yourself. But it's just a formality. They want to see if you are serious enough about the surgery to do all they are asking you to do! I followed their plan to see all the people and go to the support groups got all the signatures, etc. . I didn't do well following the diet. I lost and gained the same six pounds about 3 times. I ended up gaining by the end of the program. But just know that when they send your stuff in, you will probably get approved. I was approved in less than 24 hours after they sent my paperwork in to insurance. Hang in there! It will come to an end if you just stick with it. The most important things are to see the nutritionist and your doctor! Make sure though that you get to at least a few of the support meetings and Dr. Frye's group. As I said, you'll be amazed how much this will
prepare you for the surgery.

felt like this. Dr. Dyer always encouraged me to post to the site and
now, 10 months later, I finally have. I forgot to mention that my
husband had RNY 3-15-05 and started at 358lbs. size 6x scrubs and is
now 221lbs. and size XL scrubs. He has been such a big support to me
and an encourager. Not everyone has the blessing of having their
spouse that has gone thru it also. I tell ya there were some days I
don't see how he tolerated me. The only reason why he did is that he
understood. We are going to post some before and after pics soon.
Would anyone be interested in starting a support group in Hermitage at
Summit Medical Center, where I work in surgery? Let me know and I will
get one of the class rooms for at least once a month if not twice a
month. The meetings will have to be on a Monday or Tuesday evening and
or Saturday mornings. Give me some feedback and let me know what you
think. It is just so hard for me and my husband to make it downtown at
Centennial to the support group meetings and have not been for quite
some time now. Thanks again for everyone's support. It really means a
whole lot!!

Ohhhhh Cathy....I must admit when Dr. Houston's office gave me the date, I was too excited to really think about it. Then when I got back to IL and started to talk about it, I went HMMMMMMMMMM, this is scarey. Then I thought oh welllll, I got more guts than brains so I forged on......waiting for 'that' day to arrive.

And add to the list of problems....there was the esophageol hernia which took Dr. Houston an EXTRA 2 plus hours to repair with a total of 4 1/2 hrs. of surgery on 6/6/06 another YIKES....and I had a 6 day hospital stay compared to the normal 2 days.....another YIKES!

Here's the deal....I am NOT doing a thing =; on 7/7/07 =;

So my friend, I thank God every day for His many blessings and for my WONDERFUL new group of WL friends.

I've got to tell you... I've ALWAYS shuddered every time I read the date of your surgery! (I remember the first time I heard it and thought, "Uh-uh, not me, no way!" Do you suppose the date had anything to do with your 4 EDGs and the insurance comp. denying you AFTER they had approved it already? LOL Anyway, you are braver than me!

Ohhhhh Kathy....this website is running hot and cold....I don't know who is getting what....but I am sending emails off right and left....hoping someone out there is getting them. I was pouting earlier....see my pic [-( as hardly anyone was replying to my emails.
See I am a little spoiled O:)
I had my surgery on 6/6/06 with Dr. Houston by my side....isn't that a good date? Hmmmmmmm.
Now Kathy when your emails come up, I will definitely make sure I reply & hopefully you'll get them....just because I like you. It is my hope this mess gets taken of soon.

Is anyone else out there getting duplicate or triplicate emails like me? I think I am getting alot of emails, but they are all the same!!! Not all, but you know what I mean, some of them are the same. I think I'm reading government forms or something.

Thanks for the tip about journalling! I'm having thoughts about whether I could handle changes. My history is that I don't handle change well. But I've been working toward the surgery for almost a year. I'm going into this with my eyes WIDE OPEN! I think that journaling does help. I haven't been doing it in a long time but think I'm going to start so I can watch my progress mentally and physically. I guess I could use my OH profile page too to journal.

Hon, I felt the exact same way when I had to do the six month thing. But if you do your best to "follow the rules" you'll be amazed at the things that you find out about yourself. But it's just a formality. They want to see if you are serious enough about the surgery to do all they are asking you to do! I followed their plan to see all the people and go to the support groups got all the signatures, etc. . I didn't do well following the diet. I lost and gained the same six pounds about 3 times. I ended up gaining by the end of the program. But just know that when they send your stuff in, you will probably get approved. I was approved in less than 24 hours after they sent my paperwork in to insurance. Hang in there! It will come to an end if you just stick with it. The most important things are to see the nutritionist and your doctor! Make sure though that you get to at least a few of the support meetings and Dr. Frye's group. As I said, you'll be amazed how much this will
prepare you for the surgery.

I go to the 3rd Monday night meetings whenever I can but had forgotten
about the other 2 Monday nights. Are they the same format each Monday
or does she have a different subject each time? Or, are the Monday
nights at the center more like the Thursday and Friday small groups?

DEAR April.....when I found my collar bone about a month ago, it had been missing for ONLY 59 years. I truly know what you are talking about. I was so excited that Jim thought something was wrong.

and Friday from 12 to 1:30 ish. The last message had a typo and I put
that it started at 10:00. I didn't even catch it. Thanks, Donna, for
your feedback. Also, Dr. Frye has Monday evening meetings on the
second ,third, and fouth Mondays. The 3rd Monday Meeting is in the 3rd
floor Tower Classroom. The second and fourth Mondays are held here in
the Center clasroom. All are welcome.

In the beginning, it was hard for me to tell when I was
full...sometimes I would get a feeling of pressure on my chest and I
knew to stop. I suppose too that is why we are advised to measure
our food...to keep from over doing it. As for feeling hungry, I
didn't start feeling hungry... or rather empty until I was 6-7
months out from surgery. They say enjoy your freedom from hunger and
thought of food....do it! You will eventually feel "hunger" again.
Enjoy this time while you can :-)
It takes time for your body to adjust to the very low intake of
calories... it is certainly a shock to the system. Sometimes, it
will fight to keep the weight on too. I thought at first I wasn't
losing like I should too, but eventually, I just looked in the
mirror one day and was like whoaa.. who is that?? It is certainly a
surreal experience. Also, are you taking measurements?? Some weeks
when I didn't lose pounds, I lost inches. You might want to do that
if you're not. I say if you're doing what the program advises, just
give your body a little time or if you're really worried, talk to
one of the nutritionists. That is certainly what they are there
for :-) Hope that helps a little Fran :-)

:-& do any of you have trouble knowing when you're hungry and when you're full before the yucky feeling starts? i threw :-& up for a week and then i got ok and today i feel yucky again!!! :o) (i love these little smiles etc.) i also don't seem to be loosing weight as fast as i should right now and i'm counting cal.,carbs.,prot.,and fats! anyone know what i'm doing wrong?

Sadly, I'll NEVER see 118lbs. I'm 5'10" with an extra large frame. I'd have to lose another 86lbs to get down to 118... Considering the doc told me to get to 180 and that I could go as low as 160 if I wanted (but not to lose more than that)... sigh. No teeny tiny for me.
